FEE SCHEDULE

Initial visit (45 - 60 mins) history gathering, structural exam, and treatment, $250.00

Follow up visit (up to 30 mins), $175.00

Initial visit for children between 2 years old and 16 years old, $175.00

Follow up visit for children between 2 years old and 16 years old, $125.00

Office visit for children less than 2 years old, $100.00

Home visit, add $25.00 to the bill

Please take care of your bill at the end of each visit. Cash or check is accepted.

MISSED APPOINTMENTS

Please cancel your appointment within a 24 hour window. Otherwise, a cancellation fee will be added to your account.

HOURS

By appointment only. Same-day appointments and weekend appointments available.

INSURANCE

A form will be provided at the end of each visit. You may use this form to submit to your insurance company.

PACKAGE

Treatment throughout pregnancy limited to no more than once weekly, one postpartum visit, and treatment of newborn, $500.00.
